Chelsea forward Eden Hazard was rewarded for his hard work throughout the course of the season.

The Belgian international star bagged the award for the most assist provider in the Premier League this season.

Hazard was presented with the award shortly after he came off the bench during the goalless draw with hosts Leicester City on the final day of the Premier League season.

It was a fantastic end to the season for Eden Hazard who could have played his last game for Chelsea in the Premier League.

The former Lille attacker is expected to leave Chelsea for Spanish giants Real Madrid this summer.

The 28-year-old has one year left on his current deal at Chelsea and he’s not expected to sign a new contract at the club.

Chelsea might need to sell him this summer or risk losing him for free when his contract expires at the end of next season.

Real Madrid are expected to rebuild their playing squad in a massive way and Hazard is likely to be a part of it.

Eden Hazard was unable to add to his assist record for the season against the Foxes but his 15 assists were enough to win the award ahead of Bournemouth midfielder Ryan Fraser (14 assists).
