Chelsea are expected to demand around £100m for him.
Eden Hazard joined Chelsea from French Ligue 1 club Lille in 2012 and he has won one Europa League title, one FA Cup, two Premier League titles and one Carabao Cup title.
He has scored 16 goals in the Premier League this term in addition to the provision of 13 assists for his teammates.
